ELIGIBILITY
Eligibility for services from the Huron County Board of Developmental Disabilities is determined a little differently for each of several age groups, and is the responsibility of the Community Outreach Specialist for those aged three and up.
The Ohio Department of Developmental Disabilities provides one assessment tool for individuals between the ages of 6 and 15, and another for those aged 16 and older. Ages 16 and up: Current confirmation from a qualified clinician of the diagnosis of a severe, chronic disability, not solely caused by mental illness, manifested prior to the age of 22, and likely to continue indefinitely. After this diagnosis verification is received by HCBDD, along with all other required documentation, an assessment (the Ohio Eligibility Determination Instrument, or OEDI) will be scheduled and completed. |
The results of the assessment must show that the individual has a substantial functional limitation (SFL) in at least THREE or more of the following areas:
