SERVICES FOR CHILDREN
Eligibility for services from the Huron County Board of Developmental Disabilities is determined a little differently for each of several age groups, and is the responsibility of the Community Outreach Specialist for those aged three and up.
Birth to age 3 (Early Childhood Services and Help Me Grow): See "Early Childhood" under "Services for Children" tab for more information. Ages 3 to 5: Current confirmation from a qualified clinician of a diagnosed condition, not solely caused by mental illness, that results in at least TWO developmental delays (or an established risk). The disability causes the child to need a combination and sequence of special, interdisciplinary, or other type of care, treatment or provision of services for an extended period of time that is individually planned and coordinated for him or her. |
Ages 6 to 15: Current confirmation from a qualified clinician of the diagnosis of a severe, chronic disability, not solely caused by mental illness, that is likely to continue indefinitely.
After this diagnosis verification is received by HCBDD, along with all other required documentation, a Children's Ohio Eligibility Determination Instrument (C/OEDI) assessment will be scheduled and completed. The results of the C/OEDI must show that the individual has a substantial functional limitation (SFL) in at least THREE or more of the following six life areas:

SERVICES FOR TRANSITION YOUTH & ADULTS
Eligibility for services from the Huron County Board of Developmental Disabilities is determined a little differently for each of several age groups, and is the responsibility of the Community Outreach Specialist for those aged three and up.
The Ohio Department of Developmental Disabilities provides an assessment tool for individuals from the age of 16 through adulthood; the Ohio Eligibility Determination Instrument (OEDI). Ages 16 and up: Current confirmation from a qualified clinician of the diagnosis of a severe, chronic disability, not solely caused by mental illness, manifested prior to the age of 22, and likely to continue indefinitely. After this diagnosis verification is received by HCBDD, along with all other required documentation, an OEDI assessment will be scheduled and completed. |
The results of the OEDI must show that the individual has a substantial functional limitation (SFL) in at least THREE or more of the following seven life areas:
